State Rejection Reason

Validation Error: Declaration and signature authorization is incorrect

RuleNum: 05265

Issue: The state does not require you to send them a signature form.

Resolution: Remove the State Signature Form. Resubmit your State Return.

  1. Navigate to Review & File Forms -> Step 2. E-File Attachments
  2. Click on the link to the specific state signature form to determine if it should be filed with your return.
    1. If the Signature Form mentions "Do not mail or Do not file" then make sure not to submit it with your return.



  3. Delete/Remove the State Signature Form
  4. Resubmit your State Return.
    1. Make sure to unlock the Form in Step 1, fill in the E-File checkbox, re-run the e-file validation tool, then resubmit the form. You can reattach the Signature Form (for your records) once the State Return has been accepted.
