How Do I Add 'Date Arizona Operations Began' on the AZ 99T?

Issue: The client is unable to select / input a date for Date Arizona operations began: (Form 99T Page 1). The date picker is disabled.

Resolution: The client needs to input the date in the Arizona's General Information section.

How to Resolve:

1. Navigate to State Module -> States -> Arizona -> General Information
2. Input the date for Year Began Doing Business:
