October 10, 2025 How to Activate Form 4797 Navigate to Tax Forms -> Form 990-T -> Schedule A Tab. Navigate to Tax Forms -> Form 990-T -> Schedule A Tab. Check the box to activate Form 4797. Click the "Save" button in the Tools bar. Form 4797 is now active under Tax Forms -> Form 990-T.
